We are seeking a dynamic DIRECTOR OF NURSING SERVICES to lead clinical care and services provided in Friendsview’s private pay only, RCF licensed health services neighborhoods and care at home program. This person oversees the planning, development, implementation, coordination, evaluation and maintenance of clinical services in accordance with federal, state and local regulations as well as best practices and Friendsview’s philosophy of person-centered care. He or she collaborates with the Health Services Administrator, providing assistance in the functions of the Health Services Department and supervises the professional and paraprofessional nursing and caregiving team.

Qualified applicants will have a passion for excellence, a commitment to staff empowerment and resident-directed care and a drive to continuously improve processes and practices. A strong nursing background with a BSN or MSN, or an RN from an accredited nursing program and Oregon RN license is required. Administrative experience in long-term care or community-based care preferred.

Essential Functions:
Job Objective/ Summary: Ultimately responsible for the quality of care for residents living in one of Friendsview’s Health Services licensed care areas, or receiving in-home care services through Friends At Home. Primary focus is overseeing the planning, development, implementation, coordination, evaluation, and maintenance of nursing services (objectives, policies, procedures, protocols, systems, standards, etc) in accordance with federal, state and local regulations, and Friendsview’s philosophy of person-centered care. Collaborates with the Health Services Administrator, providing assistance in the functions of the Health Services Department. Supervises the professional and paraprofessional nursing/caregiving team.

Systems of Care

  • Ensures residents physical, social, emotional and spiritual needs are met through person-centered care in collaboration with resident, his/her family, physician, health services staff and the interdisciplinary care team.
  • Directs and ensures the department is functioning in compliance with federal, state and local standards and regulations (including HIPAA and risk management), and that standards of quality care are being followed.
  • Provides leadership and collaborates with the Health Services Leadership Team to ensure the delivery of nursing care according to physicians orders, care plans and established standards and policies to ensure quality care is provided.
  • Designs, implements, maintains, and evaluates nursing systems (including nursing care objectives and standards of nursing care practices) ensuring quality and consistent delivery of care.
  • Leads Health Services Quality Assurance and Process Improvement program according to organizational need and best practices. Develops and ensures maintenance of the quality assurance program that includes required logs and reports, responses to audits and surveys with appropriate documentation and communications.
  • Assures ongoing data collection, analysis, review and assessment of healthcare trends in the Friendsview Community, evaluates possible improved outcomes through correction and systems change.
  • Provides oversight to the Falls Prevention team, Safe Resident Handling Team, and Infection Control program, and other safety issues to ensure the on-going safety and health of residents and staff.
  • Ensures that all processes related to resident care are effective and communicated. Ensures quality assurance issues are identified and addressed, that performance improvement projects are developed, implemented, assessed for effectiveness, refined, and followed.
  • Provides oversight, assistance and guidance to the Health Services staff regarding the investigation of resident incidents, providing follow-up investigations, review of resident care interventions, and plans for identified safety hazards; as well as recording incidents, errors, or accidents. Engages Administrator in determining appropriate follow up. Ensures follow-up is in a timely manner.
  • Ensures that residents are free from abuse, mistreatment and neglect. Assures residents of a comfortable, clean and safe environment.
  • Ensures nursing/caregiving staff awareness and compliance with Federal and State regulations relative to resident rights issues.
  • Provides leadership and collaborates with the interdisciplinary team to optimize residents health and wellness, quality of life, and satisfaction. May be called upon to modify resident service plans, assist with the provider order process, complete assessments, attend care conferences, administer medications, provide treatments, triage emergency situations, interact with families, pharmacists, physician office staff, home health, and others to ensure a residents needs are met; and promotes good facility public relations during those interactions.etc.
  • In collaboration with the Health Services Leadership Team, assists the Health Services Administrator in identifying and resolving departmental deficiencies with implemented corrective action plans.
  • Adheres to Friendsview’s policies and procedures, including those covering privacy and security in compliance with HIPAA regulations, and codes of conduct. Maintains confidentiality and respects residents privacy at all times in accordance with HIPAA regulations.

Staff Leadership

  • Plans, develops and supervises the daily work flow of staff providing health care and nursing services to Health Services residents through collaboration with the Health Services Leadership Team. Serves as a resource for staff.
  • Delegates work as appropriate based on competencies and qualifications.
  • Collaborates with the Administrator and Health Services Leadership Team to identify, plan, organize, and deliver staff on-boarding, orientation, and in-service trainings.
  • Collaborates with Health Services Leadership team to assist Administrator in addressing staffing issues such as turnover, hiring, scheduling issues and effectiveness, overtime issues, corrective actions, etc.

Health Services Department Leadership & Communication Expectations:

  • Serves as a member of the Organizational Leadership Team.
  • Provides leadership to Health Services staff that is consistent with Friendsview’s mission, values and values-based performance standards.
  • Ensures departmental practices maintain high morale and staff retention, to include effective communication, prompt problem resolution, positive supervisory practices, and a positive work environment.
  • Follows up with appropriate staff, residents, or other individuals regarding reported complaints, problems, and concerns.
  • Understands, complies with, and promotes all rules and regulations regarding residents rights; and promotes positive relationships with residents, visitors and regulators.
  • Complies with, supports, and enforces all policies involving safety and infection control procedures to include safe resident handling for employees and residents.
  • Promotes good facility public relations during professional interactions.

Other Duties & Responsibilities:

  • Demonstrates the values of Friendsview Retirement Community: integrity, stewardship, compassion, community, excellence, dignity, service, Friends heritage.
  • Attends and participates in orientation, training, educational activities, staff meetings and in-services as required.
  • Provides phone consultation to FRC staff (charge nurse, caregiver, med aid, etc) as needed, potentially during off shift hours. May cover for licensed staff in a critical need.
  • Follows all safety rules and requirements, including proper resident transfers and assists as per Resident Safe Handling Program requirements.
  • Knows and follows Universal Precautions at all times.
  • Responds to and acts appropriately in emergency or disaster situations.
  • Monitors for environmental safety hazards.
  • Contributes to preparations for annual budgets.
  • Other duties as assigned.

Qualifications and Job Requirements:

Job Education, Licenses/Certifications, Knowledge & Experience:

  • Bachelor of Science in Nursing, Associate Degree in Nursing, or a Diploma in Nursing from an accredited nursing program.
  • Current, unencumbered Registered Nurse license in the state of Oregon.
  • Minimum three years of responsible nursing experience, preferably in long-term care; and minimum two years relevant experience in a clinical or similar setting, with management or supervisory experience.
  • Experience in geriatric nursing, including in ALF/RCF, is preferred.
  • Thorough understanding of federal and state rules and regulations pertinent to RCF and In-Home Care licensing. Prior experience as a DNS preferred.
  • Knowledgeable in the development and use of care plans/service plans, medication management systems, infection control practices, nurse delegation practices, person centered care and residents rights.

Supervising (direct reports and level of supervision): May provide daily work direction to direct service positions as well as Wellness Partners, Nurse Consultants and RCMs; may participate in making decisions regarding hiring, disciplining, and terminating staff; completes and communicates performance appraisals for assigned staff in collaboration with the Administrator.

Decision Making: Works with minimal supervision. Decision-making involves weighing complex factors and engaging with/interacting with others to solve problems. Guidance and direction are available upon request. The scope of decision-making responsibilities is such that decisions or judgments have the capacity to adversely impact operations.

Interpersonal Skills/Interactions: Communicates regularly with employees, residents, family members, and medical professionals. Collaboration and cooperation are necessary. Interpersonal skills are a significant aspect of the job responsibilities, and skills must be highly refined. Interactions are regularly non-routine, complex, and may require wide-ranging discussion or explanation. Regular interactions with others may be complex, demanding, difficult or disagreeable. Formal presentations may be frequent.

Must demonstrate computer skills necessary to perform all essential functions. . Demonstrated knowledge of/competence with Microsoft Word and other Microsoft Office programs. Know or be able to learn database management programs. Must be able to create reports and analyze data.

Additional Requirements:

  • Must successfully pass the Department of Human Services (DHS) criminal background check and, if applicable, fingerprinting.
  • Must be able to pass a pre-employment drug screen.
  • Must be able to pass the pre-employment reference checks.
  • Must be able to provide the official grade transcripts of the education level provided on the employment application.
  • Must be able to provide a copy of his/her own social security card for W-4 documentation.
  • Must be able to provide applicable documentation(s) for I-9 in order to establish identity and authorization to work in the US.
  • Must have valid drivers license, personal automobile liability insurance, and acceptable driving

Physical Requirements and Working Conditions:

  • Regularly sits in front of a computer screen, keyboarding for up to 2 hours at a time.
  • Must be able to use good body mechanics to move safely during the day including: moderate lifting (20 lbs.), carrying, bending, stooping, and kneeling.
  • Moderate periods of standing or walking.
  • Must possess sight and hearing senses within normal limits.
  • Infrequently, must be able to assist with one and/or two person transfer of residents, following guidelines of the Safe Resident Handling Program.
